A Fracture Coordination Unit is a multidisciplinary service designed to prevent, treat, and properly manage osteoporotic fractures, also known as fragility fractures.

The Unit



A Fragility Fracture Management Unit has as its main objective to reduce the risk of new fractures and falls through a comprehensive and multidisciplinary approach, ensuring the prescription and adherence to osteoporosis treatment, and providing the patient with tools to improve their mobility.

Goals
This unit consists of a multicenter care program, meaning it is formed by several healthcare centers working together to provide continuous and multidisciplinary care to patients suffering from fragility fractures. There are different types of fragility fractures such as hip fractures, vertebral fractures, proximal humerus fractures, distal radius fractures, and pelvic branch fractures. This initiative allows coordination between different teams, specialties, and institutions with the goal of ensuring continuity of care for patients diagnosed with fragility fractures.
- Proactively identify patients with fragility fractures in any setting: hospitalization, emergency, and outpatient care to ensure a high-quality bone assessment and appropriate follow-up.
- Initiate the appropriate osteoporosis treatment following the latest clinical guidelines to minimize the appearance of new fractures.
- Coordinate long-term follow-up to assess treatment adherence and adjust therapies according to the patient's response, reducing the risk of fractures.
- Ensure that patients receive a comprehensive fracture risk assessment, refracture risk, and bone health evaluation, including bone densitometry (DXA) tests and bone remodeling marker analysis.
- Promote patient education on bone health and fall prevention, including recommendations on nutrition, calcium and vitamin D supplementation, and strength and balance exercises.
- Reduce repeated hospitalizations and complications from new fractures through early intervention and multidisciplinary coordination (such as Rheumatology, Internal Medicine, Geriatrics, Pharmacy, or Physical Medicine and Rehabilitation).

The Capture the Fracture gold recognition is the highest distinction awarded by this global program, supported by the International Osteoporosis Foundation, which aims to facilitate the implementation of coordinated and multidisciplinary care models for secondary fracture prevention. This recognition is awarded to Fracture Liaison Services (FLS) that meet the excellence standards set in the program's Best Practice Framework.

The Internal Medicine Service provides specialized hospital care in general internal medicine to adult patients.

The Emergency Service is equipped to respond to any type of emergency 24 hours a day.

The Orthopedic Surgery and Traumatology Service treats patients with orthopedic, traumatic, or congenital conditions.

The Rheumatology Service treats patients with chronic inflammatory non-surgical diseases that affect the musculoskeletal system.

The Physical Medicine and Rehabilitation Service treats patients to help them achieve their maximum level of autonomy, functional capacity, and quality of life.

The professionals of the Pharmacy Service actively participate in the pharmacological treatment of patients.
